Lost or Swallowed Tooth Affidavit

Form TF-5 · Affidavit of Missing Tooth

Teeth go missing. They are swallowed with sandwiches, lost in sandpits and dropped down sink drains. The Department of Teeth has a long-standing procedure for this: a sworn affidavit describing the circumstances, signed by the child, so the claim can be processed without the tooth. It's official, reassuring and gives a worried child something useful to do.

Questions from grown-ups

Lost or Swallowed Tooth Affidavit

Plain answers about this document, for the grown-ups doing the filing.

Q.1Will the Tooth Fairy still come if my child swallowed their tooth?

In the Bureau's universe, yes. The affidavit is the official procedure for missing teeth, and the Department of Teeth honours every properly filed claim.

Q.2Should I worry about a swallowed baby tooth?

We're a paperwork bureau, not a medical one. A swallowed baby tooth usually passes without trouble, but if your child has pain, keeps coughing or has any difficulty breathing, contact your doctor straight away.

Q.3Can I use the affidavit for a tooth the dentist kept?

Yes. Dentist extractions are a recognised circumstance. Describe what happened in the form and the Department will take it from there.
