Costume Inspection Certificate

Form PRS-2 · Certificate of Costume Inspection

Before any trick-or-treating can begin, costumes must pass official inspection. The Parental Revenue Service's certificate confirms that your child's costume meets the highest standards of spookiness, sparkle and general excellence, with an award category, the Bureau seal and signature lines.
